New perspectives for integrated care: How healthcare coordination within the VIVA Aare network supports patients and GP practices

A recent article on Medinside shows how healthcare coordination within the VIVA Aare network is helping to specifically reduce the growing complexity of the Swiss healthcare system.

In an interview with Natyra Rexhepi, healthcare coordinator in the VIVA Aare network, it becomes clear that, particularly in the case of chronic conditions and complex treatment pathways, more is needed today than just medical excellence – coordination, guidance and proactive support are essential.

Healthcare coordination was established to support patients throughout their entire care pathway, identify gaps in care at an early stage and streamline administrative and organisational processes for GPs. General practice remains clearly at the heart of this: healthcare coordination is intended as a supportive complement, not a replacement.

The article highlights how integrated care, through clear points of contact, structured communication and targeted prevention, can reduce duplication in the long term and make better use of existing resources.

With the VIVA Aare network, Swiss Medical Network is thus sending a strong signal in favour of a modern healthcare model that rethinks care: patient-centred, networked and sustainable.
